Online Form 1 - Application for Licensure
All applicants for licensure must initially submit Form 1 along with the appropriate licensure and first registration fee. You must answer all questions and provide all information requested unless otherwise indicated. Failure to accurately complete all required parts of the application will delay its review. Additional forms below are required based on the licensure requirements of the profession. Do not use Form 1 to renew your existing license.

Form 2 - Certification of Professional Education
Documentation of professional education should be sent by your educational institution to either the State Education Department or to NCARB for review, pursuant to the directions provided below. Transcripts and the appropriate forms must be submitted directly by the educational institutions you attended. This form will not be accepted if submitted by the applicant.
If you were educated outside of the U.S. and if you want your education to be considered towards licensure, you must have Form 2 submitted by your foreign educational institution to the State Education Department. Additionally, you must submit a copy of your diploma/degree in the original language in the Additional Information section under Additional Documentation when completing the online Form 1. This is the minimum documentation required for your education evaluation. Additional documents such as detailed program information and course syllabi may be required for some applicants.
If you are attending an NAAB-accredited program and want to take the ARE while enrolled at your educational institution, please have Form 2 submitted directly by your educational institution with a partial transcript attached to Form 2.
If you are applying for licensure via the endorsement or practical examination pathway, Form 2 is not required, nor is an education evaluation needed.
The Form 2 should be completed as follows:
- Section I: Complete this section of the form before sending it to your educational institution.
- Section II: The Registrar or appropriate educational institution official must complete this section and return the form directly to the Office of the Professions at the address at the end of the form together with an official transcript. If required to complete Form 2, transcripts are required from each educational institution attended.
Submission to NCARB If you were educated within the U.S. and if you want your education to be considered towards licensure, you must have your official transcript sent to NCARB. Please see NCARB’s website at www.ncarb.org for details.
Form 3 - Verification of Out-of-State Licensure, Registration and/or Examination
This form is for applicants seeking licensure by endorsement or the practical examination and for applicants seeking a limited permit. It is not required for applicants seeking licensure by NCARB Certificate. Also, you must complete this form if you hold, or have ever held, a license or certificate to practice any profession* in any jurisdiction.
This form must be submitted directly by the licensing authority. This form will not be accepted if submitted by the applicant.
- Section I: Complete this section of the form before sending it to the jurisdiction.
- Section II: The jurisdiction must complete this section and return the form directly to the Office of the Professions at the address at the end of the form.
NOTE: A Form 3 is not required for licenses/certificates issued by the New York State Education Department. Also, if the certification/verification documentation provided by a U.S. jurisdiction contains the same information requested in the Form 3, completion of a Form 3 for that jurisdiction is not necessary. For certification/verification documentation that does not contain the same information, a Form 3 will need to be completed for that jurisdiction.
*Profession is defined as professional titles licensed under New York State Education Law.
Form 4 - Applicant Experience Record
Use this form only for experience you wish to be considered by the State Board for Architecture that is not already verified in your NCARB Record, if you need additional experience duration beyond what is verified in your NCARB Record, or if you are asked to do so by the State Board for Architecture.
You must provide one form for each supervisor for which you are seeking credit. If the required amount of experience has been obtained in one place and under one supervisor, only one form is needed. The person under whose direct supervision and responsible control you were working should complete the form and mail it directly to the return address on the form.
This form must be submitted directly by a supervisor who will verify your experience. This form will not be accepted if submitted by the applicant.
- Section I: Complete this section of the form before sending it to your supervisor.
- Section II: The supervisor must complete this section and return the form directly to the Office of the Professions at the address at the end of the form.
Form 4E - Attestation of Professional Practice for Endorsement Applicants Post-Licensure Experience Only
Use this form only if you meet the endorsement requirements and only for experience that occurred post-licensure and that you wish to be considered by the State Board for Architecture.
This form must be submitted directly by a an architect who will endorse your experience. This form willl not be accepted if submitted by the applicant.
You must provide one form for each endorser where you have been employed and for which you are seeking credit in order to show evidence of at least five years of licensed experience in architecture following initial licensure and within the seven years immediately preceding your application for licensure. If the required amount of experience has been obtained in one place and under one endorser, only one form is needed. The person endorsing your post-licensure experience should complete the form and mail it directly to the return address on the form.
The Form 4E must be completed as follows
- Section I: Complete this section of the form before sending it to the architect who will endorse your experience.
- Section II: The architect must complete this section and return the form directly to the Office of the Professions at the address at the end of the form.
Form 4L - Attestation of Lawful Practice Outside New York State as an Architect
Use this form only if you are seeking a limited permit to practice architecture on a single project in New York in your lifetime.
This form must be submitted directly by three architects who will attest to your lawful practice as an architect outside of New York State. This form will not be accepted if submitted by the applicant. You must provide one form to each of three architects who can attest to your lawful practice.
- Section I: Complete this section of the form before sending it to the architect who can attest to your lawful practice.
- Section II: The architect must complete this section and return the form directly to the Office of the Professions at the address at the end of the form.
Form 4P - Attestation of 10 Years of Lawful Practice Outside New York State as Principal/Shareholder/Owner of Firm
This form must be used for Principal/Practical Examination Candidates ONLY.
A Form 4P must be submitted directly by three architects who can attest to your lawful practice as principal/shareholder/owner of your own architecture firm outside New York State. This form will not be accepted if submitted by the applicant.
- Section I: Complete this section of the form before sending it to the architect who can attest to your lawful practice.
- Section II: The architect must complete this section and return the form directly to the Office of the Professions at the address at the end of the form.
Form 5 - Application for Limited Permit for Non-Resident Architect
All applicants for a limited permit must complete this form and submit it with the $347 fee directly to the Office of the Professions at the mailing address at the end of Form 5. Make checks payable to the New York State Education Department. Your cancelled check is your receipt.
You will need a Form 3 - Verification of Other Professional Licensure/Certification and/or Examination submitted on your behalf by the licensing authority that issued your license and whose seal you will be using on the project. Form 3 must be submitted directly by the licensing authority to the Office of the Professions at the address at the end of the form. This form will not be accepted if submitted by the applicant.
You will also need a Form 4L - Attestation of Lawful Practice Outside New York State as an Architect submitted on your behalf by three licensed and registered architects who will attest to your lawful practice. These architects MUST submit the Form 4L directly to the Office of the Professions at the address at the end of the form. This form will not be accepted if submitted by the applicant.
