The New York State Education Department (“SED”) Office of the Professions (“OP”) is alerting everyone to a vishing scam that has been brought to our attention. Phishing—or “vishing”—scams impersonate SED employees or websites attempting to collect licensure and personal information from the licensee. If you receive an inquiry which you believe to be suspicious do not provide any information. To verify if the inquiry was from OP, contact OP directly using the following contact information. You may report suspicious calls or emails to the Federal Trade Commission.

Important Notice: DO NOT use Form 1 if you are already licensed in this profession in New York State. A New York State professional license is valid for life unless it is revoked, annulled, or suspended by the Board of Regents. To practice in New York State, your professional license must be registered. To renew your registration online, visit http://op.nysed.gov/services/online-registration-renewal. If your registration has lapsed for longer than 4 months, submit a Delayed Registration Application.

All applicants for licensure must initially submit Form 1 along with the $430 licensure and first registration fee. You must answer all questions and provide all information requested unless otherwise indicated. Failure to accurately complete all required parts of the application will delay its review. Additional forms below are required based on the licensure requirements of the profession. Do not use Form 1 to renew your existing license.

Continuing Your Application
If you have started an application within the past 30 days, and have not yet completed it, you can use this link to continue your application. You will need your Application ID, Social Security Number, and Date of Birth.

Use this form to apply for a New York State Intern Geologist Certificate. Qualifications for New York State Intern Geologist Certificate may be found on our web site under License Requirements.

Applicants must complete this form and submit it with the appropriate fee ($70 only) directly to the Office of the Professions at the mailing address at the end of the Form. Make checks payable to the New York State Education Department. NOTE: Your cancelled check is your receipt.

Section I: Complete this section and be sure to sign and date item 8.
Section II: Send the entire form to the educational institution(s) you attended and ask that they complete Section II and return all pages of the form along with any required documentation directly to the Office of the Professions at the address at the end of the form.
  • Be sure to ask your school to attach an official transcript or marksheet if you completed a program not registered by the New York State Education Department as licensure qualifying.
  • This form must be submitted directly by the educational institution. The Office of the Professions will not accept this form if submitted by the applicant.

Electronic Education Documentation

The Office of the Professions (OP) will accept official electronic transcripts and forms from educational institutions (i.e. colleges/universities) or designated third-party* transcript entities located in the United States, Canada, and the Philippines provided that:

  • The transcript is the certified true and official academic record and the document does NOT have an expiration date**.
  • OP can independently verify that the documentation is received directly from the educational institution’s registrar or officially designated third-party.
  • If a third-party transcript provider is involved, it is clear that the educational institution has designated the third party as the official sole provider of its transcripts.
  • The applicant had no opportunity to directly access or alter the transcript before it is sent or transmitted.
  • Any educational institution education documentation submissions should be made electronically to DPLSEduc@nysed.gov***.

*OP will only accept third-party submissions after we have determined that the arrangement between the educational institution and the third party is consistent with our security and verification standards.

**Transcript documents with expiration dates cannot be accepted. Expirations on links to the document are acceptable.

  1. Use this form to verify licensure in another jurisdiction. This jurisdiction should be:
    • Your primary place of practice. If this jurisdiction does not license geologists then
    • The jurisdiction of original licensure.
    Complete section I of this form and be sure to sign and date item 9. Then send this   form to the jurisdiction and ask that they complete section II and return the form directly to the Office of the Professions.
  2. Use this form to verify that you passed the Fundamentals of Geology examination and/or the Practice of Geology examination in another jurisdiction. Complete Section I of this form and be sure to sign and date item 9. Then send this form to the jurisdiction(s) from which you wish to have examination grades transferred or to ASBOG, and ask that they complete section II and return the form directly to the Office of the Professions.

Note: A Form 3 is not required for licenses/certificates issued by the New York State Education Department. Also, If the certification/verification documentation provided by a U.S. jurisdictions contains the same information requested in the Form 3, completion of a Form 3 for that jurisdiction is not necessary. For certification/verification documentation that does not contain the same information, a Form 3 will need to be completed for that jurisdiction.

Electronic Verification of Licensure, Certification and/or Examination

The Office of the Professions (OP) will accept electronic verifications of licensure, certification and examination completion from other licensing authorities located in the United States provided that:

  • OP can independently authenticate that the verification is received directly from the licensing authority.
  • The applicant had no opportunity to directly access or alter the verification before it is sent or transmitted.
  • Any licensing authority verification submissions should be made electronically to DPLSVerif@nysed.gov.*

Please note: Section II of Form 4A must be typed or printed legibly, if typed, be sure to use a font size of 10 or greater as smaller fonts create difficulty for reviewers and will slow the review of your application.   • OP can independently verify that the documentation is received directly from the supervisor

New York State Institutions/Schools - ONLY

For those New York State institutions/schools who wish to register their professional geology educational programs, with the New York State Education Department as a licensure qualifying program, please use this link to the memorandum outlining curricular requirements.

Registration as a licensure qualifying program means that students that meet the requirement of the Commissioner’s Regulations 68.9(d.)(1) – [are within 20 credits of graduation from a bachelor’s degree program in geological sciences registered as leading to licensure pursuant to section 52.46 of this Title] are eligible to sit for the ASBOG Fundamentals of Geology Examination (FG) after submitting an applicant and obtaining approval by NYSED Office of Professions. Colleges and universities with licensure qualifying programs are listed on the NYSED web page in the Inventory of Registered Programs and can be easily accessed by potential students doing their college searches.

Certification of Completion for NYS Institutions/Schools whose baccalaureate program in Geological Sciences is Registered with the Department as Licensure Qualifying

These forms are for use by New York State Institutions/Schools only. Not to be used by individuals. These forms must be submitted directly by appropriate personnel from the educational institution. The Office of the Professions will not accept this form if submitted by the applicant/student.

Certification of Completion for NYS Institutions/Schools whose baccalaureate program in Geological Sciences is Registered with the Department as Licensure Qualifying

For use by New York State Institutions whose baccalaureate programs(s) in Geological Sciences are specifically registered by the NYS Education Department as Licensure Qualifying. This form may be used by schools in lieu of individual Form 2s for students who have completed 3 years and are within 20 semester credits or 30 quarter credits of graduation in a program in “Geologic Sciences” qualified by the New York State Education Department as licensure qualifying, to sit for the ASBOG “Fundamentals of Geology” (FG) exam in New York State.

This form may be used by schools in lieu of individual Form 2s for students who have graduated from a baccalaureate program in Geological Sciences that qualifies them, after successful completion of the ASBOG Fundamentals of Geology (FG) examination, for a Certificate as an "Intern Geologist".
