Online Form 1 - Application for Licensure
All applicants for licensure must initially submit Form 1 along with the $372 licensure and first registration fee. You must answer all questions and provide all information requested unless otherwise indicated. Failure to accurately complete all required parts of the application will delay its review. Additional forms below are required based on the licensure requirements of the profession. Do not use Form 1 to renew your existing license.
Continuing Your Application
If you have started an application within the past 30 days, and have not yet completed it, you can use this link to continue your application. You will need your Application ID, Social Security Number, and Date of Birth.

Form 2 - Certification Of Professional Education
This form must be submitted directly by the professional school.
- Section I: Complete this section of the form before sending it to your professional school. If you graduated from a veterinary school not accredited by the American Veterinary Medical Association (AVMA), notify the school that a transcript must accompany the form. A description of the structure and contents of the clinical portion of the veterinary program may also be needed.
If you have attended more than one academic institution, copy the form and send one to each institution.
If your education was completed outside the United States, attach to your Application for Licensure (Form 1) a copy of your secondary school diploma or certificate, and your preprofessional and professional credentials (transcript and diploma). Each document not printed in English must be accompanied by an original acceptable translation that includes all written and printed matter on the original and has been translated by a qualified translator.
NOTE: Translations that do not meet Education Department requirements may be referred back to the applicant.
- Section II: The registrar, dean, rector, or principal of the veterinary school must complete the appropriate parts of this section and return the form directly to the Office of the Professions in an official school envelope at the address on the form.
Please make as many copies of Form 2 as needed.
Form 2PPE - Certification of Pre-Professional Education
This form must be submitted directly by the college you attended prior to your veterinary studies.
Complete the form and send it to the college(s) you attended before you began your veterinary studies. Ask the college to attach an official transcript to the form and mail both to the Office of the Professions at the address provided on the form. If you have attended more than one academic institution, provide a copy (photocopy or print from the web) of the form to each institution.
Form 2CT - Certification Of Clinical Training
This form must be submitted directly by the institution where you received your clinical training.
Use this form only if you received your professional degree from a program not accredited or approved by the New York State Education Department and your clinical experience for the degree was completed in a country other than where the school was located. You must have Form 2CT completed by the institution where your clinical training was completed. Form 2CT must be sent by the institution(s) directly to the address at the bottom of the form.
NOTE: An Educational Commission for Foreign Veterinary Graduates (ECFVG) Certificate may be submitted in lieu of Form 2CT.
Form 3 - Verification Of Out-Of-State Licensure, Registration, And/Or Examination
This form must be submitted directly by the state licensing authority.
Use this form ONLY if you are licensed in another state and have not attempted the CCT examination. Applicants who have passed the NBE and CCT or have passed NAVLE and whose education has been approved by New York State are not required to file Form 3 or 4.
- Section I: Complete this section of the form before sending it to the state licensing authority. Be sure to include any fee required by the other state.
- Section II: The appropriate official of the licensing authority must complete this section and return the form directly to the Office of the Professions at the address on the form.
Note: A Form 3 is not required for licenses/certificates issued by the New York State Education Department. Also, If the certification/verification documentation provided by U.S. jurisdictions contains the same information requested in the Form 3, completion of a Form 3 for that jurisdiction is not necessary. For certification/verification documentation that does not contain the same information, a Form 3 will need to be completed for that jurisdiction.
Form 4 - Verification Of Professional Practice
This form must be submitted directly by the licensed veterinarian who completed the form.
Use this form ONLY if you are licensed in another state and have not attempted the CCT examination. Applicants who have passed the NBE and CCT or have passed NAVLE and whose education has been approved by New York State are not required to file Form 3 or 4.
- Section I: Complete this section of the form before sending it to the licensed veterinarian verifying your practice. More than one form may be necessary to verify the total number of years of professional practice required for endorsement.
- Section II: A duly licensed veterinarian in good standing in the state where the applicant is licensed must complete this section and return the form directly to the Office of the Professions at the address on the form.
Online Form 5 - Application For Limited Permit
All applicants for a limited permit must initially submit Form 5 along with the $105 limited permit fee. You must also submit a Form 5CS - Certification of Supervisor for Limited Permit along with your application.

Changes to Supervisors and/or Settings
If you change supervisors and/or settings, or you have to add or remove a supervisor or setting, after a permit is issued, you must obtain an amended permit using the Limited Permit Change Form. After changes are processed you will receive an amended permit. A fee is not required for an amended permit issued as a result of a change in supervisor or setting. You must also submit a Form 5CS - Certification of Supervisor for Limited Permit for each new supervisor you are adding (see Upload Additional Documentation above).
Form 5CS - Certification of Supervisor for Limited Permit
This form is not for all applicants. Use this form ONLY if you are applying/have applied for a New York State Limited Permit as a Veterinarian online.
- Section I: Complete this section.
- Section II: Your supervisor must complete this section before you can submit it to the Office of the Professions at the appropriate address at the end of the form. It is your responsibility to ensure your supervisor fully completes Section II. Failure to complete this form will delay its review.
Form COSN - Attestation of Veterinarian's Provision of Free Spaying and/or Neutering Services for Continuing Education Credit
This form must be used if you are a veterinarian seeking to satisfy a portion of your continuing education requirements by providing free spaying and/or neutering services. For more information about this option, see Continuing Education. If you are not claiming the provision of free spaying and/or neutering services for continuing education credit, you do not need to fill out this form.
This form does not need to be submitted to the Department but it must be retained, along with any documentation attached to it, for a minimum of 6 years from the date of the provision of the services claimed on it. Once completed, it is recommended that you keep this form at your practice location. You are required to provide it, along with any attached documentation, to the New York State Education Department upon request.
