The New York State Education Department (“SED”) Office of the Professions (“OP”) is alerting everyone to a vishing scam that has been brought to our attention. Phishing—or “vishing”—scams impersonate SED employees or websites attempting to collect licensure and personal information from the licensee. If you receive an inquiry which you believe to be suspicious do not provide any information. To verify if the inquiry was from OP, contact OP directly using the following contact information. You may report suspicious calls or emails to the Federal Trade Commission.

How to Obtain Replacement Registration and Licensure Documentation

Registration Certificate

The registration certificate is a light green document with a darker green border which has your address and lists the period during which you are registered to practice. If you are practicing in New York State, your registration must be renewed periodically.

To obtain a replacement Registration Certificate, submit an Online Replacement Registration Certificate Form (There is a $10 fee for each replacement certificate)

License Parchment

Your parchment is a unique, diploma-like document issued only once, when you are initially licensed. It is permanently valid unless revoked, annulled, or suspended by the Board of Regents. The license parchment is printed on diploma-like paper which has the date of licensure on it. Professionals in New York State can only have one parchment for each licensed profession.
