Below you will find more on how to update personal information with the Office of the Professions. Such updates include changes to your: address, name, email address, citizenship/immigration status, and social security number as well as notifying the Office of the Professions about a deceased applicant.
Updating your address:
- Please submit an Address Change Form or Chat to change or confirm your mailing address on file with the Department.
- Once you are licensed, Education Law requires that you notify the Department of any change in your mailing address within 30 days of the change. Failure to do so may be considered professional misconduct. It may also delay renewal and result in late fees to renew the registration of a professional license.
Updating your name:
- Please complete our online Name Change Form and attach supporting documentation.

Updating your email address:
- Please submit an Email Address Change Form to change or confirm your email address on file with the Department.
Updating your citizenship/immigration status:
- Complete the Statement of Citizenship/Immigration Status Form and mail it to the address at the end of the form. Allow 2-4 weeks for processing.
Updating your social security number:
- Please submit an Update of Social Security Number Form. You will also need to submit a copy of a social security card with the updated number and a copy of a government issued photo ID.
Notifying the Office of the Professions about a deceased applicant:
- Please submit a Notification of Deceased Applicant Form and attach supporting documentation. You will need to submit a copy of an official death certificate.
